Mileage Charge Agreement
This Agreement is between Tidy Pawz, located at 170 Arrowwood Place in Malta, and the client. It establishes the terms for a mileage charge in addition to booked service fees for pet care services provided at the client's house.
Any client located more than 10 miles (GPS-driven distance) from Tidy Pawz in Malta will be charged a $15 mileage fee.
By signing, the client acknowledges and agrees to the terms and conditions of this Mileage Charge Agreement.
LOCKBOX AGREEMENT
This Agreement is made between the client and Tidy Pawz Mobile Peticures regarding the use of a lockbox to store keys or access codes.
The client provides keys or access codes to Tidy Pawz Mobile Peticures for storage in the lockbox.
Tidy Pawz Mobile Peticures owns the lockbox, which we will use to provide pet nail services while the client is not home.
The client ensures keys or codes are accurate.
Tidy Pawz Mobile Peticures is not liable for lost or stolen keys/codes.
The Client indemnifies Tidy Pawz Mobile Peticures from any liability.
Lockbox is for specified property keys/codes only.
The client shall not disclose the lockbox location or combination to others.
The client must notify Tidy Pawz Mobile Peticures of lost or compromised keys/codes.
If the lockbox is not returned in working order, the client will be charged the cost of replacing it.
Tidy Pawz Mobile Peticures can terminate the Agreement for any Client breaches at any time.
Tidy Pawz Nail Care Agreement-DOGS ONLY
This agreement is made between Tidy Pawz, the "Company," and the dog owner, hereinafter referred to as the "Client."
The Client agrees to maintain a regular schedule of nail care for their dog(s) with Tidy Pawz. The recommended schedule is every 4-6 weeks, depending on the dog's individual needs. Failure to adhere to this schedule may result in discomfort or injury to the dog and additional fees for longer nails.
The Client understands that regular nail care is essential for the health and well-being of their dog(s). Overgrown nails can cause pain, discomfort, and difficulty walking the dog. By maintaining a regular schedule of nail care, the Client ensures the overall health and happiness of their dog(s).
The Client agrees to notify Tidy Pawz of any changes to their schedule or availability that may affect their ability to maintain the recommended nail care schedule. Tidy Pawz will try to accommodate the Client's needs and reschedule appointments as necessary.
If the Client repeatedly fails to adhere to the recommended nail care schedule, Tidy Pawz reserves the right to deny services. This decision will be made at the discretion of Tidy Pawz and communicated to the Client in writing.
By signing, the Client acknowledges that they have read and understand the importance of regular nail care for their dog(s) and agree to adhere to the recommended schedule outlined above.
Tidy Pawz Arrival Window Agreement
This agreement is made between Tidy Pawz, the "Company," and the pet owner, hereinafter referred to as the "Client."
The Client understands that Tidy Pawz operates on a 2-hour appointment arrival window. This means that Tidy Pawz will provide the Client with a 2-hour time frame in which the appointment is expected to occur. For example, if the appointment is scheduled for 10:00 AM, the arrival window may be between 9:00 AM and 11:00 AM.
The Client acknowledges that Tidy Pawz optimizes routes a week before the appointment to provide efficient and timely service to all clients. This means that clients will not receive their specific time slot until the routes have been finalized. Tidy Pawz will communicate the arrival window to the Client as soon as it is determined.
The Client agrees to be available during the 2-hour arrival window provided by Tidy Pawz. If the Client cannot accommodate the scheduled appointment time, they must notify Tidy Pawz as soon as possible to reschedule.
Tidy Pawz will try to adhere to the agreed-upon arrival window, but unforeseen circumstances, such as traffic or weather conditions, may affect the scheduled appointment time. Tidy Pawz will communicate any delays or changes to the Client in a timely manner.
The Client is advised to disregard any specific time the booking system provides during the booking process, as the arrival window will be communicated within the week before the appointment date.
By signing, the Client acknowledges that they have read and understood Tidy Pawz's 2-hour arrival window policy and agree to be available during the provided time frame for their scheduled appointment.
Incident reporting agreement
This is to inform you about the necessary steps and reporting requirements in the event of an animal bite incident involving your pet. We must establish a clear understanding and agreement regarding reporting such incidents to ensure the safety and well-being of all parties involved.
If an animal bite occurs while I am caring for or assisting your pet, I will promptly notify you. Please acknowledge and understand the importance of being informed about such incidents. Your cooperation will enable us to take appropriate measures to address potential risks or concerns.
Additionally, as mandated by state regulations, reporting any animal bites to the relevant authorities is essential. In accordance with these regulations, I will be required to complete a county form and submit it accordingly. This reporting process ensures compliance with legal requirements and helps to safeguard the health and safety of both humans and animals.
I assure you that any information shared with the authorities, including your pet's vaccination records, will be treated with the utmost confidentiality and used solely to comply with legal reporting obligations.

Guidelines for Pet Grooming
Only pets that are healthy and fit can be groomed. Grooming on elderly or sick pets will be at the owners' risk as it may expose pre-existing health or skin conditions for which Tidy Pawz Mobile Pet-icures cannot be held liable.
My pet's rabies vaccine must be current unless otherwise discussed, as required by law.
In case of an emergency, Tidy Pawz Mobile Pet-icures may have to contact the nearest Veterinarian and authorize the Vet to treat my pet as necessary at my expense.
Payment should be made at the time of service. Cash, digital payment, or credit cards are accepted. Tidy Pawz Mobile Pet-icures also accepts PayPal, Venmo, and ApplePay as payment methods.
Cancellation and no-show policies
I should provide 24 hours notice to cancel or reschedule my appointment to avoid being charged the FULL fee.
If the cancellation or rescheduling is due to weather conditions, a family emergency, or any other uncontrollable circumstance, Tidy Pawz can waive the fee within 24 hours.
Tidy Pawz Mobile Pet-icures reserves the right to cancel or reschedule a groom if necessary, and every effort will be made to reschedule at a time convenient for both the client and the groomer.
A "no-show" will be charged the FULL fee as it causes a revenue loss. To avoid such situations, I should call and cancel or reschedule when possible.
Additional guidelines
Tidy Pawz Mobile Pet-icures reserves the right to muzzle any dog anytime for everyone's safety.
Tidy Pawz Mobile Pet-icures also reserves the right to refuse service to any pet or client for any reason.
“By signing, I acknowledge all the guidelines mentioned above and understand and agree to release Tidy Pawz Mobile Pet-icures from any and all liabilities, expenses, and costs (including veterinary and legal fees) resulting from any service provided, or unintentional injury to my pet while under their care or afterward. I also grant Tidy Pawz Mobile Pet-icures permission to groom my pet(s).”
Credit card on file agreement
We have a policy that requires all TIDY PAWZ clients to keep a credit card on file for payment purposes. Our system lets us keep your credit card information on file, which we can only use under the terms you specify below. By providing us with your credit card information, you are giving TIDY PAWZ permission to automatically charge your credit card on an as-needed basis (or if payment is not made by you within 7 days of an invoice) for the amounts due for services received, providing you with a convenient and hassle-free payment process. Any canceled or missed appointments without a 24-hour notice will result in the credit card on file being charged the late cancellation/no-show fee for a minimum of $25.
If the credit card information we have on file changes for any reason, you must notify TIDY PAWZ as soon as possible. If you have any questions about a charge, please let us know within 15 days. After 30 days, all charges will be assumed to be correct. We will maintain a clear and transparent record of all payments and charges, ensuring you are always informed and in control of your financial transactions with us.
If a charge is declined, you will be asked for a new credit card number and/or payment before your next appointment.
“I HAVE READ, UNDERSTAND, AND AGREE TO ABIDE BY THE CREDIT CARD ON FILE AGREEMENT AND AUTHORIZE TIDY PAWZ TO SAVE AND CHARGE MY CREDIT CARD AS OUTLINED IN THIS AGREEMENT.”
